HOUSEHOLD GOODS (for Automobile and/or Motorcycle Claims, See Below)
The Platinum Plus Program is available primarily through moving and storage companies or international forwarders. All insured shipments have an identifying certificate number. If you are filing a claim and have this number, please provide it on the claim form. If you don't have the certificate number, complete the balance of the information and provide the name of your mover / forwarder. We can request missing information directly from them. Your claim form may be mailed or faxed to us.
Please be sure to provide your current contact information so we can reach you if necessary.
AUTOMOBILES AND/OR MOTORCYCLES
If you moved and insured your automobile and/or motorcycle, please submit one form for your household goods and a separate form for your automobile and/or motorcycle.
